Welcome Class of 2028!Many of the UHS requirements for undergraduate matriculation will be completed online through MyUHS, a secure student health portal. To log in, you will use your Princeton NetID which will be emailed to you around May 15.You may also wish to print the UHS Requirements Checklist now, to help you track completion of the requirements described below.In addition to the requirements, there is important health-related information you should familiarize yourself with before coming to campus. Now Make an appointment with your health care provider You must make an appointment with your primary health care provider (e.g., nurse practitioner, doctor, physician's assistant) to get a physical examination and a copy of your immunization records. It could take time to get an appointment, so we recommend contacting your health care provider's office as soon as possible so they can inform you on the best timing and method (in-office visit or telehealth appointment). If you do not have an identified clinic or primary care provider, email [email protected] for help connecting to resources that may be available to you. Read the Athletic Medicine Welcome Letter (for varsity or club rugby athletes only) Varsity or club rugby athletes only: Read the letter (.docx) and other important information for NCAA qualified athletes (.docx). At Health Care Provider's Appointment Obtain necessary immunizations and a copy of your record Print a copy of the Required and Recommended Immunizations for Princeton University. Due to the history of meningitis B outbreak at United States colleges (including ours in 2013-2014), the Meningitis B vaccine is recommended to all incoming students. Read more about vaccination recommendations.Be sure that your immunization record includes all of the immunization requirements. If you get them at UHS, you may be charged fees. Your health care provider may also share copies of lab titer results or chest x-ray reports as part of your immunization record. Obtain medical clearance Print a copy of the Medical Clearance form and obtain your health care provider's signature. Provide proof of Sickle Cell testing and a copy of results (for varsity or club rugby athletes only) Varsity or club rugby athletes only: at your health care provider appointment, obtain an order for a Sickle Cell Trait test and request a copy of your results. Obtain copies of medical records (i.e., lab results, x-ray reports, surgical history), if requested (for varsity or club rugby athletes only) Varsity or club rugby athletes only: if you have any significant medical or surgical history (e.g., heart murmur, dizziness, notes from musculoskeletal evaluation, operative/surgical notes, MRI, x-ray or other test reports), you must upload records to MyUHS before arriving on campus. We may require copies of surgical consultations and diagnostic test results for clearance. By June 30 Make your health insurance decision Compare your healthcare coverage to the Student Health Plan (SHP) and decide whether you will enroll in the SHP or opt-out. Varsity or club rugby athletes only: read important information about insurance coverage. Complete your health history form Log in to MyUHS and complete your Health History form. Varsity or club rugby athletes only: also complete the Incoming Athlete Health History form. Enter immunization history into MyUHS In the MyUHS portal, manually enter your immunizations, including any new immunizations you received at your health care provider’s appointment. Obtain NCAA Medical Exemption for ADHD/ADD (for varsity or club rugby athletes only) Varsity or club rugby athletes only: if you are taking a stimulant medication (i.e., adderral, concerta, Ritalin) for treatment of Attention Deficit Hyperactivity Disorder (ADHD) or Attention Deficit Disorder (ADD), print the NCAA Medical Exemption form and have your prescribing provider complete it. Upload forms to MyUHS In the MyUHS portal, upload your Immunization Record and Medical Clearance Form.Varsity or club rugby athletes only: also upload the results of your Sickle Cell Trait test and NCAA Medical Exemption Form for ADHD/ADD (if applicable) and any other medical records (if requested). Upload letters from your health care provider, if applicable If indicated by your health history, upload to MyUHS letters from your health care provider explaining any chronic conditions or major surgical procedures (e.g., cardiac evaluation for a heart murmur, even if the murmur was determined to be innocent) that might affect your health while at Princeton. (If applicable) Provide permission for health care of a minor If you are under the age of 18 at the time of your arrival on campus, your parent or guardian must grant Permission for Health Care of a Minor, and upload the form to the MyUHS portal. Upload or complete any additional forms If indicated in MyUHS or by email, upload or complete any additional forms necessary. (Optional) Enroll in the Dental Plan (Optional) Enroll in the Vision Plan Before Coming to Campus Bring health insurance cards If you chose to opt-out of the Student Health Plan, bring your private insurance cards with you. Indicate communication preferences Complete the Texting Opt-in/Opt-out form in MyUHS to opt-in to receive text reminders for appointments and additional communication from UHS. Complete Self Identification Form for New Jersey Complete the Self Identification Form for New Jersey in MyUHS for point of care/lab testing at UHS, where you are required to either self-identify your race, ethnicity, sexual orientation and gender identity, or select "prefer not to disclose". We require this information, when conducting point of care testing in our facility, in order to be compliant with state regulatory requirements.
