Welcome Incoming Graduate Students!Many of the UHS requirements for incoming graduate students are completed through MyUHS, a secure online student health portal. You will need your Princeton NetID to access MyUHS.If you are a Degree-seeking student, try accessing the portal after you receive your welcome letter via email from UHS.If you are a nondegree student, including GVSRC status, you will be able to access MyUHS approximately 21 days after you have confirmed your acceptance with Graduate Admissions.You should complete all requirements 1 month before arrival to campus or by the date indicated below – whichever occurs first. You may also wish to print the Incoming Grad Checklist now, to help you track completion of the requirements described below.In addition to the matriculation requirements, there is important health-related information you should familiarize yourself with before coming to campus. Now Make an appointment with your health care provider You must make an appointment with your primary health care provider (e.g., nurse practitioner, doctor, physician's assistant). It could take time to get an appointment, so we recommend contacting your health care provider's office as soon as possible so they can inform you of the best timing and method (in-office visit or telehealth appointment). If you do not have an identified clinic or primary care provider, email [email protected] for help connecting to resources that may be available to you. At your healthcare provider appointment Obtain necessary immunizations and a copy of your record Print a copy of the Required and Recommended Immunizations for Princeton University. Be sure that your immunization record includes all of the immunization requirements. Your provider may also share copies of lab titer (blood test) results showing immunity or chest x-ray reports as part of your immunization record. An immunization hold will be applied to your academic registration until your immunizations are on file with UHS. By June 30 (or 1 month before arrival on campus) Enter immunizations and upload immunization record Log in to MyUHS to manually enter your immunizations data by selecting the "Immunizations" menu item. Then upload a copy of the official immunization record. We accept documents from prior schools, universities, pharmacies, medical providers, public health departments – they must have your name and a second legal identification (e.g., date of birth) directly on the document. Complete your Health History Form Log in to MyUHS and complete your Health History Form by selecting the "Forms" menu item. (If applicable) Upload letters/documents describing medical conditions from your healthcare provider If indicated by your health history, upload to MyUHS letters/documents from your health care provider explaining any chronic conditions or major surgical procedures (e.g., cardiac evaluation for a heart murmur, even if the murmur was determined to be innocent) that might affect your health while at Princeton. Review Student Health Plan (SHP) Insurance coverage You are automatically enrolled in the Princeton University Student Health Plan (SHP) when you pay your tuition. Learn more about your coverage. Complete optional insurance elections Complete your election to enroll in the dental plan, vision plan, and/or dependent* plans. To enroll, log in to MyUHS and go to the Insurance tab. Complete form to enroll in UHS services (for Graduate Exchange Scholars only) Graduate Exchange Scholars only: Although you are ineligible for enrollment in the Student Health Plan (SHP), you may purchase a UHS health plan for services provided at Frist Health Center. Review the enrollment form. Explore all of UHS’ services available under this plan Complete the Self-Identification form for New Jersey Complete the Self Identification Form for New Jersey in MyUHS for point of care/lab testing at UHS, where you are required to either self-identify your race, ethnicity, sexual orientation and gender identity, or select "prefer not to disclose". We require this information, when conducting point of care testing in our facility, in order to be compliant with state regulatory requirements. *Dependent eligibility is defined as a legally married spouse and/or child under the age of 26
