As we start the 2023-24 plan year, the following serves as an overview of key plan benefits and other pertinent information about the Student Health Plan and the optional dental and vision plans. Learn more about: Plan Coverage and Benefits Finding a Provider Referrals ID Cards Filing Claims Plan Coverage and Benefits Student Health Plan (Aetna) $200 annual individual plan deductible ($400 for family). After the deductible has been met for the year, eligible medical expenses are covered at 90% in-network or 70% out-of-network. Mental health is covered at 100% in-network with $10 co-pay per visit, 70% out-of-network with deductible applied, or 100% with a $20 per visit co-pay for EPN. For additional plan information, review the Student Health Plan. Prescription Plan (Optum Rx) $100 annual individual plan deductible ($200 for family). After the deductible has been met for the year, eligible prescription expenses are covered at the retail (pharmacy) or mail order co-pay level. For plan information, review the Prescription Plan. Dental Plan (Cigna) – Optional plan In-network routine dental exam and cleaning covered at 100% with $0 co-pay (twice per plan year/every 6 months). You must use the in-network primary dental office you assigned. To change your primary dental office or find which office you assigned, contact Cigna. For plan information, review the Dental Plan. Vision Plan (VSP) – Optional plan Well-vision/routine eye exam covered at 100% with $10 co-pay once per plan year. Provides coverage towards the cost of eyeglasses, contact lenses, and lens enhancements. Gives extra savings towards eyeglasses and sunglasses. For plan information, review the Vision Plan. Finding a Provider Student Health Plan (Aetna) Find an in-network medical or mental health provider at Find Provider. Find an EPN mental health provider at Find EPN Provider. Dental Plan (Cigna) Find an in-network dental provider by going to the Cigna website and select the Cigna Dental Care Access Plus plan. Vision Plan (VSP) Find an in-network vision provider by going to the VSP website. Referrals Referrals for off-campus specialist services are required effective September 5, 2023. Referrals must be initiated through a University Health Services (UHS) clinician prior to your off-campus visits. See pages 5-6 of the SHP Summary Guide(.docx) for referral requirements and exceptions. This applies to students and adult dependents who are enrolled in the Student Health Plan. Pediatric referrals are not required. ID Cards Plastic ID cards will not be mailed unless you contact the vendor directly to request one. You may access your insurance cards by going to ID Cards. Filing a claim for services provided by UHS For billable services done at UHS (such as immunizations), the cost is charged upfront to your student account. You would pay for the charge via your student account and then file a claim with the Aetna Student Health plan for reimbursement of that expense. Reimbursement would be mailed to your campus mailbox in the form of a check. Reimbursement would not be applied directly back to your student account.
